Esta vez, se trataba de un jugador de baloncesto de 24 a\u00f1os, con entrenamientos casi diarios, que ven\u00eda arrastrando molestias en la parte baja de la espalda. Lo interesante del caso es c\u00f3mo, a pesar de no haber una lesi\u00f3n grave, el dolor interfer\u00eda cada vez m\u00e1s con su rendimiento y calidad de vida.<\/p><p class=\"\" data-start=\"712\" data-end=\"971\">Desde el primer encuentro, supe que se trataba de una <strong data-start=\"766\" data-end=\"803\">lumbalgia mec\u00e1nica por sobrecarga<\/strong>, una afecci\u00f3n com\u00fan pero muchas veces mal entendida. Hoy quiero contarte c\u00f3mo lo abordamos paso a paso y qu\u00e9 herramientas usamos para lograr una recuperaci\u00f3n completa.<\/p><h2 class=\"\" data-start=\"973\" data-end=\"1029\">\u00bfPor qu\u00e9 aparece la lumbalgia en j\u00f3venes deportistas?<\/h2><p class=\"\" data-start=\"1031\" data-end=\"1364\">No hace falta sufrir una ca\u00edda o un golpe para sentir dolor lumbar. En este caso, el paciente refer\u00eda molestias que se intensificaban al flexionar el tronco o despu\u00e9s de entrenamientos intensos. Nada llamativo en los estudios por im\u00e1genes, salvo una leve <strong data-start=\"1286\" data-end=\"1313\">protrusi\u00f3n discal L4-L5<\/strong>, algo relativamente frecuente en personas activas.<\/p><p class=\"\" data-start=\"1366\" data-end=\"1545\">La clave estaba en entender qu\u00e9 estaba generando esa sobrecarga repetitiva: <strong data-start=\"1442\" data-end=\"1544\">mala distribuci\u00f3n del esfuerzo, falta de estabilidad central y patrones de movimiento ineficientes<\/strong>.<\/p><h2 class=\"\" data-start=\"1547\" data-end=\"1595\">Evaluaci\u00f3n cl\u00ednica: m\u00e1s all\u00e1 de la resonancia<\/h2><p class=\"\" data-start=\"1597\" data-end=\"1983\">Durante la evaluaci\u00f3n, detectamos una clara contractura en la musculatura paravertebral, limitaci\u00f3n en la movilidad lumbar y un patr\u00f3n de flexi\u00f3n lumbar excesiva al agacharse, con poca participaci\u00f3n de la cadera. No hab\u00eda signos neurol\u00f3gicos ni de compromiso estructural severo, por lo que descartamos patolog\u00edas m\u00e1s complejas como una <strong data-start=\"1933\" data-end=\"1954\">espondilolistesis<\/strong> o disfunciones sacroil\u00edacas.<\/p><p class=\"\" data-start=\"1985\" data-end=\"2121\">La conclusi\u00f3n fue clara: est\u00e1bamos frente a una <strong data-start=\"2033\" data-end=\"2056\">lumbalgia funcional<\/strong>, generada por sobrecarga mec\u00e1nica y deficiente control postural.<\/p><hr class=\"\" data-start=\"2123\" data-end=\"2126\" \/><h2 class=\"\" data-start=\"2128\" data-end=\"2200\">Tratamiento por fases: c\u00f3mo se estructura una rehabilitaci\u00f3n efectiva<\/h2><p class=\"\" data-start=\"2202\" data-end=\"2431\">Abordar este tipo de dolor requiere de una planificaci\u00f3n que respete los tiempos del cuerpo. Dividimos la rehabilitaci\u00f3n en cuatro etapas, adaptando la carga y las estrategias terap\u00e9uticas en funci\u00f3n de la evoluci\u00f3n del paciente.<\/p><h3 class=\"\" data-start=\"2433\" data-end=\"2493\">Fase 1 \u2013 Aguda: controlar el dolor y la tensi\u00f3n muscular<\/h3><p class=\"\" data-start=\"2495\" data-end=\"2601\">El objetivo en los primeros d\u00edas fue reducir el dolor y liberar la zona lumbar sin generar m\u00e1s irritaci\u00f3n.<\/p><ul data-start=\"2603\" data-end=\"2874\"><li class=\"\" data-start=\"2603\" data-end=\"2680\"><p class=\"\" data-start=\"2605\" data-end=\"2680\"><strong data-start=\"2605\" data-end=\"2620\">Crioterapia<\/strong> localizada, aplicada durante 15 minutos post-entrenamiento.<\/p><\/li><li class=\"\" data-start=\"2681\" data-end=\"2767\"><p class=\"\" data-start=\"2683\" data-end=\"2767\"><strong data-start=\"2683\" data-end=\"2719\">Terapia manual descontracturante<\/strong>, especialmente sobre paravertebrales y gl\u00fateos.<\/p><\/li><li class=\"\" data-start=\"2768\" data-end=\"2874\"><p class=\"\" data-start=\"2770\" data-end=\"2874\"><strong data-start=\"2770\" data-end=\"2795\">Vendaje neuromuscular<\/strong>, que ayud\u00f3 a disminuir la sensaci\u00f3n de rigidez y a mantener una mejor postura.<\/p><\/li><\/ul><p class=\"\" data-start=\"2876\" data-end=\"2939\">El alivio fue progresivo, pero se not\u00f3 desde la primera semana.<\/p><h3 class=\"\" data-start=\"2941\" data-end=\"2997\">Fase 2 \u2013 Subaguda: recuperar el movimiento sin dolor<\/h3><p class=\"\" data-start=\"2999\" data-end=\"3092\">Con los s\u00edntomas controlados, pasamos a trabajar la movilidad y la activaci\u00f3n muscular suave:<\/p><ul data-start=\"3094\" data-end=\"3318\"><li class=\"\" data-start=\"3094\" data-end=\"3208\"><p class=\"\" data-start=\"3096\" data-end=\"3208\"><strong data-start=\"3096\" data-end=\"3132\">Movilizaciones pasivas y activas<\/strong>, especialmente en cadera y columna tor\u00e1cica, para descargar la zona lumbar.<\/p><\/li><li class=\"\" data-start=\"3209\" data-end=\"3318\"><p class=\"\" data-start=\"3211\" data-end=\"3318\"><strong data-start=\"3211\" data-end=\"3242\">Ejercicios de control motor<\/strong>, como basculaciones p\u00e9lvicas, puentes y activaci\u00f3n de transverso abdominal.<\/p><\/li><\/ul><p class=\"\" data-start=\"3320\" data-end=\"3397\">Este momento fue clave para reeducar el cuerpo sin que el dolor reapareciera.<\/p><h3 class=\"\" data-start=\"3399\" data-end=\"3463\">Fase 3 \u2013 Fortalecimiento: base s\u00f3lida para prevenir reca\u00eddas<\/h3><p class=\"\" data-start=\"3465\" data-end=\"3580\">A medida que mejoraba su movilidad, fuimos incorporando ejercicios m\u00e1s exigentes para construir una base funcional:<\/p><ul data-start=\"3582\" data-end=\"3762\"><li class=\"\" data-start=\"3582\" data-end=\"3668\"><p class=\"\" data-start=\"3584\" data-end=\"3668\"><strong data-start=\"3584\" data-end=\"3603\">Trabajo de core<\/strong>, con foco en estabilidad profunda m\u00e1s que en fuerza superficial.<\/p><\/li><li class=\"\" data-start=\"3669\" data-end=\"3762\"><p class=\"\" data-start=\"3671\" data-end=\"3762\"><strong data-start=\"3671\" data-end=\"3711\">Ejercicios isom\u00e9tricos y exc\u00e9ntricos<\/strong>, como planchas, bird-dog y peso muerto modificado.<\/p><\/li><\/ul><p class=\"\" data-start=\"3764\" data-end=\"3888\">Tambi\u00e9n incluimos ejercicios de cadena cin\u00e9tica cerrada, priorizando la coordinaci\u00f3n entre tronco y extremidades inferiores.<\/p><h3 class=\"\" data-start=\"3890\" data-end=\"3959\">Fase 4 \u2013 Reintegro funcional: volver al entrenamiento con control<\/h3><p class=\"\" data-start=\"3961\" data-end=\"4083\">En esta \u00faltima etapa, buscamos que el paciente pudiera entrenar sin dolor y, sobre todo, sin sobrecargar la regi\u00f3n lumbar:<\/p><ul data-start=\"4085\" data-end=\"4293\"><li class=\"\" data-start=\"4085\" data-end=\"4186\"><p class=\"\" data-start=\"4087\" data-end=\"4186\"><strong data-start=\"4087\" data-end=\"4155\">Reeducaci\u00f3n postural durante movimientos t\u00e9cnicos del baloncesto<\/strong>, como el salto y la recepci\u00f3n.<\/p><\/li><li class=\"\" data-start=\"4187\" data-end=\"4293\"><p class=\"\" data-start=\"4189\" data-end=\"4293\"><strong data-start=\"4189\" data-end=\"4242\">Adaptaci\u00f3n progresiva a la carga de entrenamiento<\/strong>, con trabajo en conjunto con su preparador f\u00edsico.<\/p><\/li><\/ul><p class=\"\" data-start=\"4295\" data-end=\"4394\">Adem\u00e1s, le dejamos una rutina preventiva semanal con movilidad y fuerza espec\u00edfica para su deporte.<\/p><hr class=\"\" data-start=\"4396\" data-end=\"4399\" \/><h2 class=\"\" data-start=\"4401\" data-end=\"4469\">Resultados: el dolor no tiene por qu\u00e9 ser parte del entrenamiento<\/h2><p class=\"\" data-start=\"4471\" data-end=\"4655\">En solo cuatro semanas, el paciente ya entrenaba sin dolor. En ocho semanas, hab\u00eda recuperado completamente su funcionalidad, con mejores patrones de movimiento que antes de la lesi\u00f3n.<\/p><p class=\"\" data-start=\"4657\" data-end=\"4894\">Este caso me recuerda que el dolor lumbar no es exclusivo de quienes llevan una vida sedentaria. Tambi\u00e9n aparece en personas activas, muchas veces como una se\u00f1al de que es hora de <strong data-start=\"4837\" data-end=\"4893\">revisar c\u00f3mo nos movemos y no solo cu\u00e1nto entrenamos<\/strong>.<\/p><p data-start=\"4657\" data-end=\"4894\"><strong>Profesor Sebasti\u00e1n Pablo Galleano<\/strong><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div
