Eso fue lo que pens\u00f3 una paciente de 22 a\u00f1os que lleg\u00f3 a la consulta con molestias en el hombro derecho. Jugadora amateur de tenis, entrenaba cinco veces por semana y notaba que cada vez le costaba m\u00e1s realizar golpes por encima de la cabeza sin sentir dolor.<\/p><p class=\"\" data-start=\"701\" data-end=\"1016\">El caso era muy claro: se trataba de una <strong data-start=\"742\" data-end=\"791\">tendinopat\u00eda del supraespinoso por sobrecarga<\/strong>, una condici\u00f3n frecuente en deportes de raqueta o con movimientos repetitivos de brazo elevado. Pero lo interesante es c\u00f3mo logramos su recuperaci\u00f3n completa en ocho semanas, sin que tenga que dejar de entrenar por completo.<\/p><h2 class=\"\" data-start=\"1018\" data-end=\"1079\">\u00bfCu\u00e1ndo prestar atenci\u00f3n al dolor de hombro en el deporte?<\/h2><p class=\"\" data-start=\"1081\" data-end=\"1385\">El hombro es una articulaci\u00f3n compleja y, a la vez, muy expuesta en deportes como el tenis, donde se requiere fuerza, velocidad y precisi\u00f3n en rangos extremos de movimiento. Esta paciente refer\u00eda dolor al hacer saques y voleas, con una molestia punzante que aparec\u00eda sobre todo despu\u00e9s del entrenamiento.<\/p><p class=\"\" data-start=\"1387\" data-end=\"1504\">Adem\u00e1s, notaba una inflamaci\u00f3n leve y una sensaci\u00f3n de \u201cfatiga\u201d que no desaparec\u00eda del todo con el descanso habitual.<\/p><h2 class=\"\" data-start=\"1506\" data-end=\"1564\">Evaluaci\u00f3n funcional: lo que no se ve en una resonancia<\/h2><p class=\"\" data-start=\"1566\" data-end=\"1945\">En la evaluaci\u00f3n cl\u00ednica, encontramos dolor a la palpaci\u00f3n en el tend\u00f3n del <strong data-start=\"1642\" data-end=\"1659\">supraespinoso<\/strong>, debilidad en el deltoides y alteraciones claras en la mec\u00e1nica del golpeo, especialmente en la fase final del saque. Aunque la resonancia magn\u00e9tica mostr\u00f3 una <strong data-start=\"1820\" data-end=\"1859\">tendinopat\u00eda leve del supraespinoso<\/strong>, el verdadero desaf\u00edo estaba en corregir los desequilibrios que la estaban generando.<\/p><h3 class=\"\" data-start=\"1947\" data-end=\"2005\">Diagn\u00f3stico diferencial y an\u00e1lisis del gesto deportivo<\/h3><p class=\"\" data-start=\"2007\" data-end=\"2336\">Antes de avanzar con el tratamiento, descartamos otras posibilidades como el <strong data-start=\"2084\" data-end=\"2123\">s\u00edndrome de pinzamiento subacromial<\/strong> o una lesi\u00f3n m\u00e1s compleja del <strong data-start=\"2154\" data-end=\"2174\">manguito rotador<\/strong>. Pero al no haber signos estructurales graves, el diagn\u00f3stico qued\u00f3 claro: una <strong data-start=\"2254\" data-end=\"2289\">sobrecarga funcional del hombro<\/strong> por t\u00e9cnica y preparaci\u00f3n f\u00edsica no adecuadas.<\/p><h2 class=\"\" data-start=\"2338\" data-end=\"2397\">Tratamiento por fases: paso a paso hacia la recuperaci\u00f3n<\/h2><p class=\"\" data-start=\"2399\" data-end=\"2573\">Como kinesi\u00f3logo, siempre priorizo una rehabilitaci\u00f3n progresiva, adaptada a los tiempos y necesidades del deporte. En este caso, lo dividimos en cuatro fases bien definidas.<\/p><h3 class=\"\" data-start=\"2575\" data-end=\"2626\">Fase 1 \u2013 Aguda: bajar la inflamaci\u00f3n y el dolor<\/h3><p class=\"\" data-start=\"2628\" data-end=\"2720\">El objetivo fue controlar los s\u00edntomas sin detener por completo la actividad. Implementamos:<\/p><ul data-start=\"2722\" data-end=\"2999\"><li class=\"\" data-start=\"2722\" data-end=\"2783\"><p class=\"\" data-start=\"2724\" data-end=\"2783\"><strong data-start=\"2724\" data-end=\"2743\">Reposo relativo<\/strong>, evitando golpes por encima del hombro.<\/p><\/li><li class=\"\" data-start=\"2784\" data-end=\"2853\"><p class=\"\" data-start=\"2786\" data-end=\"2853\"><strong data-start=\"2786\" data-end=\"2820\">Crioterapia post-entrenamiento<\/strong>, aplicada durante 15-20 minutos.<\/p><\/li><li class=\"\" data-start=\"2854\" data-end=\"2930\"><p class=\"\" data-start=\"2856\" data-end=\"2930\"><strong data-start=\"2856\" data-end=\"2884\">Neuromodulaci\u00f3n con TENS<\/strong>, para aliviar el dolor sin uso de medicaci\u00f3n.<\/p><\/li><li class=\"\" data-start=\"2931\" data-end=\"2999\"><p class=\"\" data-start=\"2933\" data-end=\"2999\"><strong data-start=\"2933\" data-end=\"2954\">Vendaje funcional<\/strong>, brindando soporte sin inmovilizar del todo.<\/p><\/li><\/ul><p class=\"\" data-start=\"3001\" data-end=\"3098\">Esta etapa fue clave para ganar confianza y empezar a generar alivio sin frenar su rutina f\u00edsica.<\/p><h3 class=\"\" data-start=\"3100\" data-end=\"3166\">Fase 2 \u2013 Subaguda: recuperar movilidad y control neuromuscular<\/h3><p class=\"\" data-start=\"3168\" data-end=\"3259\">Con el dolor bajo control, pasamos a trabajar sobre el movimiento y la activaci\u00f3n muscular:<\/p><ul data-start=\"3261\" data-end=\"3553\"><li class=\"\" data-start=\"3261\" data-end=\"3356\"><p class=\"\" data-start=\"3263\" data-end=\"3356\"><strong data-start=\"3263\" data-end=\"3300\">Movilizaciones articulares suaves<\/strong>, buscando restaurar la movilidad sin generar molestias.<\/p><\/li><li class=\"\" data-start=\"3357\" data-end=\"3453\"><p class=\"\" data-start=\"3359\" data-end=\"3453\"><strong data-start=\"3359\" data-end=\"3406\">Ejercicios isom\u00e9tricos del manguito rotador<\/strong>, ideales para reactivar sin irritar el tend\u00f3n.<\/p><\/li><li class=\"\" data-start=\"3454\" data-end=\"3553\"><p class=\"\" data-start=\"3456\" data-end=\"3553\"><strong data-start=\"3456\" data-end=\"3476\">Terapia acu\u00e1tica<\/strong>, aprovechando la flotaci\u00f3n para reducir carga y trabajar amplitudes seguras.<\/p><\/li><\/ul><p class=\"\" data-start=\"3555\" data-end=\"3678\">Ya en esta fase, la paciente comenz\u00f3 a notar mejoras claras, especialmente en la sensaci\u00f3n de \u201cligereza\u201d al mover el brazo.<\/p><h3 class=\"\" data-start=\"3680\" data-end=\"3747\">Fase 3 \u2013 Fortalecimiento: trabajo espec\u00edfico y control de carga<\/h3><p class=\"\" data-start=\"3749\" data-end=\"3862\">A medida que progresamos, el objetivo fue fortalecer los m\u00fasculos del hombro sin caer en patrones compensatorios:<\/p><ul data-start=\"3864\" data-end=\"4076\"><li class=\"\" data-start=\"3864\" data-end=\"3964\"><p class=\"\" data-start=\"3866\" data-end=\"3964\"><strong data-start=\"3866\" data-end=\"3913\">Ejercicios exc\u00e9ntricos del manguito rotador<\/strong>, esenciales para mejorar la resistencia tendinosa.<\/p><\/li><li class=\"\" data-start=\"3965\" data-end=\"4076\"><p class=\"\" data-start=\"3967\" data-end=\"4076\"><strong data-start=\"3967\" data-end=\"4004\">Propiocepci\u00f3n y control escapular<\/strong>, utilizando bandas el\u00e1sticas, pelotas de mano y superficies inestables.<\/p><\/li><\/ul><p class=\"\" data-start=\"4078\" data-end=\"4219\">Tambi\u00e9n trabajamos con ejercicios en cadena cin\u00e9tica cerrada, como apoyo frontal y planchas modificadas, para reforzar la estabilidad global.<\/p><h3 class=\"\" data-start=\"4221\" data-end=\"4287\">Fase 4 \u2013 Reintegro funcional: volver a la cancha con seguridad<\/h3><p class=\"\" data-start=\"4289\" data-end=\"4379\">El \u00faltimo paso fue reeducar el gesto deportivo y preparar su regreso al tenis competitivo:<\/p><ul data-start=\"4381\" data-end=\"4594\"><li class=\"\" data-start=\"4381\" data-end=\"4485\"><p class=\"\" data-start=\"4383\" data-end=\"4485\"><strong data-start=\"4383\" data-end=\"4421\">Reeducaci\u00f3n biomec\u00e1nica del golpeo<\/strong>, filmando y analizando el saque para corregir errores t\u00e9cnicos.<\/p><\/li><li class=\"\" data-start=\"4486\" data-end=\"4594\"><p class=\"\" data-start=\"4488\" data-end=\"4594\"><strong data-start=\"4488\" data-end=\"4530\">Carga progresiva en los entrenamientos<\/strong>, con seguimiento cercano y ajustes en la planificaci\u00f3n semanal.<\/p><\/li><\/ul><p class=\"\" data-start=\"4596\" data-end=\"4718\">Adem\u00e1s, dejamos pautas claras de fortalecimiento preventivo y movilidad diaria para mantener los resultados a largo plazo.<\/p><h2 class=\"\" data-start=\"4720\" data-end=\"4748\">Resultados y aprendizajes<\/h2><p class=\"\" data-start=\"4750\" data-end=\"5077\">En cuatro semanas, el dolor hab\u00eda disminuido significativamente. En ocho semanas, la paciente volvi\u00f3 a jugar tenis sin dolor y con una mejora visible en su mec\u00e1nica. M\u00e1s all\u00e1 de la recuperaci\u00f3n, lo m\u00e1s importante fue que entendi\u00f3 que el entrenamiento no solo es volumen o intensidad, sino tambi\u00e9n calidad y conciencia corporal.<\/p><p
