Un claro ejemplo de esto es el caso de un paciente de 25 a\u00f1os que vino a verme por un dolor cervical persistente. Entrenaba seis veces por semana, combinando fuerza y resistencia, y hab\u00eda notado un aumento del dolor en la parte posterior del cuello, sobre todo despu\u00e9s de las sesiones m\u00e1s intensas.<\/p><h2 class=\"\" data-start=\"805\" data-end=\"855\">\u00bfCu\u00e1ndo el dolor cervical deja de ser \u201cnormal\u201d?<\/h2><p class=\"\" data-start=\"857\" data-end=\"1153\">Muchas personas que entrenan duro tienden a minimizar el dolor, pensando que es parte del proceso. Este paciente ya hab\u00eda tenido episodios previos de cervicalgia, pero esta vez notaba algo diferente: rigidez en el cuello, dolor al girar la cabeza y cefaleas tensionales que se volv\u00edan frecuentes.<\/p><p class=\"\" data-start=\"1155\" data-end=\"1310\">Despu\u00e9s de conversar y evaluar sus h\u00e1bitos de entrenamiento y postura, comenzamos con una evaluaci\u00f3n cl\u00ednica completa para entender el origen del problema.<\/p><h2 class=\"\" data-start=\"1312\" data-end=\"1366\">Evaluaci\u00f3n y diagn\u00f3stico: m\u00e1s all\u00e1 de la estructura<\/h2><p class=\"\" data-start=\"1368\" data-end=\"1814\">Durante el examen f\u00edsico encontr\u00e9 varios indicadores clave: hiperton\u00eda en los trapecios superiores, disminuci\u00f3n de la movilidad en flexi\u00f3n lateral y una alteraci\u00f3n en su postura habitual, marcada por una posici\u00f3n adelantada de cabeza. Los estudios por im\u00e1genes (radiograf\u00eda cervical) no mostraban lesiones estructurales significativas, lo que nos llev\u00f3 a un diagn\u00f3stico funcional: <strong data-start=\"1749\" data-end=\"1813\">cervicalgia por sobrecarga muscular y mala mec\u00e1nica postural<\/strong>.<\/p><h3 class=\"\" data-start=\"1816\" data-end=\"1866\">Diferencias con otras causas de dolor cervical<\/h3><p class=\"\" data-start=\"1868\" data-end=\"2205\">En estos casos, siempre considero diagn\u00f3sticos diferenciales como un esguince cervical leve o una cervicalgia mec\u00e1nica pura. Sin embargo, el patr\u00f3n cl\u00ednico y la ausencia de traumatismo nos orientaron hacia una causa funcional: la sobrecarga acumulada por entrenamientos intensos, con escasa atenci\u00f3n a la movilidad y el control postural.<\/p><h2 class=\"\" data-start=\"2207\" data-end=\"2262\">Rehabilitaci\u00f3n: una estrategia progresiva y adaptada<\/h2><p class=\"\" data-start=\"2264\" data-end=\"2460\">Como en toda patolog\u00eda de origen funcional, el abordaje debe ser personalizado y progresivo. Trabajamos por fases, respetando la evoluci\u00f3n de los s\u00edntomas y la respuesta del cuerpo al tratamiento.<\/p><h3 class=\"\" data-start=\"2462\" data-end=\"2519\">Fase 1 \u2013 Aguda: aliviar el dolor y reducir la tensi\u00f3n<\/h3><p class=\"\" data-start=\"2521\" data-end=\"2627\">Nuestro primer objetivo fue bajar la intensidad del dolor y relajar la musculatura. Para ello, utilizamos:<\/p><ul data-start=\"2629\" data-end=\"2895\"><li class=\"\" data-start=\"2629\" data-end=\"2709\"><p class=\"\" data-start=\"2631\" data-end=\"2709\"><strong data-start=\"2631\" data-end=\"2657\">Crioterapia localizada<\/strong> en zona cervical posterior para disminuir el dolor.<\/p><\/li><li class=\"\" data-start=\"2710\" data-end=\"2815\"><p class=\"\" data-start=\"2712\" data-end=\"2815\"><strong data-start=\"2712\" data-end=\"2749\">T\u00e9cnicas de liberaci\u00f3n miofascial<\/strong>, especialmente en trapecios superiores y elevador de la esc\u00e1pula.<\/p><\/li><li class=\"\" data-start=\"2816\" data-end=\"2895\"><p class=\"\" data-start=\"2818\" data-end=\"2895\"><strong data-start=\"2818\" data-end=\"2855\">Correcciones posturales iniciales<\/strong>, con \u00e9nfasis en la conciencia corporal.<\/p><\/li><\/ul><p class=\"\" data-start=\"2897\" data-end=\"2993\">En esta etapa, muchas veces con peque\u00f1os ajustes ya se empieza a notar una mejora significativa.<\/p><h3 class=\"\" data-start=\"2995\" data-end=\"3067\">Fase 2 \u2013 Subaguda: recuperar movilidad y comenzar el fortalecimiento<\/h3><p class=\"\" data-start=\"3069\" data-end=\"3155\">Una vez que el dolor disminuy\u00f3, avanzamos hacia una segunda fase en la que trabajamos:<\/p><ul data-start=\"3157\" data-end=\"3364\"><li class=\"\" data-start=\"3157\" data-end=\"3248\"><p class=\"\" data-start=\"3159\" data-end=\"3248\"><strong data-start=\"3159\" data-end=\"3196\">Movilizaciones articulares suaves<\/strong>, buscando restaurar rangos de movimiento sin dolor.<\/p><\/li><li class=\"\" data-start=\"3249\" data-end=\"3364\"><p class=\"\" data-start=\"3251\" data-end=\"3364\"><strong data-start=\"3251\" data-end=\"3288\">Ejercicios isom\u00e9tricos cervicales<\/strong>, ideales para activar la musculatura estabilizadora sin generar irritaci\u00f3n.<\/p><\/li><\/ul><p class=\"\" data-start=\"3366\" data-end=\"3473\">Adem\u00e1s, empezamos a incluir pautas para mejorar la higiene postural durante el d\u00eda y en sus entrenamientos.<\/p><h3 class=\"\" data-start=\"3475\" data-end=\"3527\">Fase 3 \u2013 Fortalecimiento: estabilizar y prevenir<\/h3><p class=\"\" data-start=\"3529\" data-end=\"3609\">A medida que la movilidad mejor\u00f3, nos enfocamos en el fortalecimiento funcional:<\/p><ul data-start=\"3611\" data-end=\"3858\"><li class=\"\" data-start=\"3611\" data-end=\"3742\"><p class=\"\" data-start=\"3613\" data-end=\"3742\"><strong data-start=\"3613\" data-end=\"3663\">Ejercicios de estabilizaci\u00f3n cervical profunda<\/strong>, fundamentales para sostener la cabeza sin sobrecargar m\u00fasculos superficiales.<\/p><\/li><li class=\"\" data-start=\"3743\" data-end=\"3858\"><p class=\"\" data-start=\"3745\" data-end=\"3858\"><strong data-start=\"3745\" data-end=\"3766\">Trabajo escapular<\/strong>, especialmente con ejercicios de retracci\u00f3n escapular y activaci\u00f3n de romboides y serratos.<\/p><\/li><\/ul><p class=\"\" data-start=\"3860\" data-end=\"4012\">Durante esta fase tambi\u00e9n ajustamos su rutina de entrenamiento para evitar ejercicios que generaban compensaciones, como el press militar mal ejecutado.<\/p><h3 class=\"\" data-start=\"4014\" data-end=\"4073\">Fase 4 \u2013 Reintegro: volver al ritmo sin repetir errores<\/h3><p class=\"\" data-start=\"4075\" data-end=\"4194\">En esta \u00faltima etapa, el objetivo fue ayudarlo a reincorporarse a su entrenamiento con seguridad y sin dolor. Para eso:<\/p><ul data-start=\"4196\" data-end=\"4441\"><li class=\"\" data-start=\"4196\" data-end=\"4324\"><p class=\"\" data-start=\"4198\" data-end=\"4324\">Hicimos un trabajo de <strong data-start=\"4220\" data-end=\"4253\">reeducaci\u00f3n postural din\u00e1mica<\/strong>, aplicando conceptos de control motor durante movimientos funcionales.<\/p><\/li><li class=\"\" data-start=\"4325\" data-end=\"4441\"><p class=\"\" data-start=\"4327\" data-end=\"4441\">Ajustamos su planificaci\u00f3n de cargas y enfatizamos la importancia del descanso y la variabilidad en los est\u00edmulos.<\/p><\/li><\/ul><p class=\"\" data-start=\"4443\" data-end=\"4557\">Tambi\u00e9n reforzamos estrategias preventivas, como el uso de rutinas de movilidad antes y despu\u00e9s del entrenamiento.<\/p><h2 class=\"\" data-start=\"4559\" data-end=\"4594\">Resultados y reflexiones finales<\/h2><p class=\"\" data-start=\"4596\" data-end=\"4980\">Despu\u00e9s de cuatro semanas, el paciente hab\u00eda reducido notablemente el dolor y la rigidez. A las ocho semanas, su movilidad y fuerza cervical eran normales y hab\u00eda vuelto a entrenar sin molestias.
