Ten\u00eda 70 a\u00f1os y una expresi\u00f3n de desconcierto m\u00e1s que de dolor. Me dijo: \u201cFue solo una ca\u00edda tonta, pero desde entonces no me animo a caminar bien. Me da miedo volver a caer\u201d. Esa frase marc\u00f3 el inicio de un proceso de rehabilitaci\u00f3n que no solo apuntaba a curar un <strong data-start=\"561\" data-end=\"584\">esguince de tobillo<\/strong>, sino a devolverle la seguridad en cada paso.<\/p><p data-start=\"632\" data-end=\"763\">En este art\u00edculo quiero contarte c\u00f3mo abordamos su recuperaci\u00f3n, paso a paso, y qu\u00e9 pod\u00e9s hacer si pas\u00e1s por una situaci\u00f3n similar.<\/p><hr data-start=\"765\" data-end=\"768\" \/><h2 data-start=\"770\" data-end=\"844\">El tobillo en adultos mayores: por qu\u00e9 una torcedura no es \u201calgo menor\u201d<\/h2><p data-start=\"846\" data-end=\"1199\">Muchas veces escuchamos que los esguinces son lesiones leves. Pero en personas mayores, incluso un <strong data-start=\"945\" data-end=\"965\">esguince grado I<\/strong> puede tener consecuencias importantes si no se trata correctamente. La combinaci\u00f3n de <strong data-start=\"1052\" data-end=\"1096\">osteopenia, hipertensi\u00f3n y edad avanzada<\/strong> puede generar un cuadro m\u00e1s complejo: menos estabilidad, mayor riesgo de ca\u00eddas y temor al movimiento.<\/p><p data-start=\"1201\" data-end=\"1280\">En el caso de Beatriz, tras una ca\u00edda dom\u00e9stica, el tobillo izquierdo mostraba:<\/p><ul data-start=\"1282\" data-end=\"1421\"><li data-start=\"1282\" data-end=\"1312\"><p data-start=\"1284\" data-end=\"1312\"><strong data-start=\"1284\" data-end=\"1312\">Dolor medial persistente<\/strong><\/p><\/li><li data-start=\"1313\" data-end=\"1349\"><p data-start=\"1315\" data-end=\"1349\"><strong data-start=\"1315\" data-end=\"1349\">Hematoma en el mal\u00e9olo interno<\/strong><\/p><\/li><li data-start=\"1350\" data-end=\"1376\"><p data-start=\"1352\" data-end=\"1376\"><strong data-start=\"1352\" data-end=\"1376\">Inflamaci\u00f3n moderada<\/strong><\/p><\/li><li data-start=\"1377\" data-end=\"1421\"><p data-start=\"1379\" data-end=\"1421\">Inestabilidad al pararse sobre un solo pie<\/p><\/li><\/ul><p data-start=\"1423\" data-end=\"1590\">Una <strong data-start=\"1427\" data-end=\"1484\">ecograf\u00eda confirm\u00f3 distensi\u00f3n del ligamento deltoideo<\/strong>, y la radiograf\u00eda descart\u00f3 fracturas. Ya ten\u00edamos el diagn\u00f3stico: <strong data-start=\"1551\" data-end=\"1589\">esguince leve del ligamento medial<\/strong>.<\/p><hr data-start=\"1592\" data-end=\"1595\" \/><h2 data-start=\"1597\" data-end=\"1653\">Primera etapa: aliviar el dolor sin perder movimiento<\/h2><p data-start=\"1655\" data-end=\"1742\">Durante la fase aguda, el objetivo fue controlar la inflamaci\u00f3n sin inmovilizar de m\u00e1s.<\/p><ul data-start=\"1744\" data-end=\"2002\"><li data-start=\"1744\" data-end=\"1818\"><p data-start=\"1746\" data-end=\"1818\">Usamos un <strong data-start=\"1756\" data-end=\"1777\">vendaje funcional<\/strong>, que le permiti\u00f3 moverse con contenci\u00f3n.<\/p><\/li><li data-start=\"1819\" data-end=\"1912\"><p data-start=\"1821\" data-end=\"1912\">Indicamos <strong data-start=\"1831\" data-end=\"1850\">reposo relativo<\/strong>, priorizando posiciones elevadas y evitando cargas excesivas.<\/p><\/li><li data-start=\"1913\" data-end=\"2002\"><p data-start=\"1915\" data-end=\"2002\">Sumamos <strong data-start=\"1923\" data-end=\"1960\">analg\u00e9sicos locales y crioterapia<\/strong>, especialmente durante los primeros d\u00edas.<\/p><\/li><\/ul><p data-start=\"2004\" data-end=\"2218\">Evitar el reposo absoluto fue clave. En pacientes mayores, quedarse quietos demasiado tiempo puede ser contraproducente. A los pocos d\u00edas, comenzamos con movilidad leve, siempre guiada y dentro del rango sin dolor.<\/p><hr data-start=\"2220\" data-end=\"2223\" \/><h2 data-start=\"2225\" data-end=\"2287\">Segunda etapa: recuperar movilidad y activar la musculatura<\/h2><p data-start=\"2289\" data-end=\"2369\">A medida que el dolor fue cediendo, empezamos a trabajar el rango de movimiento.<\/p><ul data-start=\"2371\" data-end=\"2744\"><li data-start=\"2371\" data-end=\"2484\"><p data-start=\"2373\" data-end=\"2484\">Introdujimos <strong data-start=\"2386\" data-end=\"2420\">ejercicios de movilidad pasiva<\/strong>: movimientos asistidos para mantener la flexibilidad articular.<\/p><\/li><li data-start=\"2485\" data-end=\"2604\"><p data-start=\"2487\" data-end=\"2604\">Luego avanzamos a <strong data-start=\"2505\" data-end=\"2525\">movilidad activa<\/strong>, con movimientos lentos y controlados, como escribir el abecedario con el pie.<\/p><\/li><li data-start=\"2605\" data-end=\"2744\"><p data-start=\"2607\" data-end=\"2744\">Tambi\u00e9n incluimos <strong data-start=\"2625\" data-end=\"2653\">ejercicios circulatorios<\/strong>, como flexi\u00f3n plantar y dorsiflexi\u00f3n, para mejorar el retorno venoso y disminuir el edema.<\/p><\/li><\/ul><p data-start=\"2746\" data-end=\"2851\">Lo importante en esta fase fue que Beatriz pudiera volver a mover su tobillo sin temor ni compensaciones.<\/p><hr data-start=\"2853\" data-end=\"2856\" \/><h2 data-start=\"2858\" data-end=\"2928\">Tercera etapa: fortalecimiento y estabilidad para prevenir reca\u00eddas<\/h2><p data-start=\"2930\" data-end=\"3007\">Una vez que la movilidad mejor\u00f3, comenzamos con el fortalecimiento funcional.<\/p><ul data-start=\"3009\" data-end=\"3381\"><li data-start=\"3009\" data-end=\"3090\"><p data-start=\"3011\" data-end=\"3090\">Empezamos con <strong data-start=\"3025\" data-end=\"3065\">ejercicios de resistencia progresiva<\/strong> usando bandas el\u00e1sticas.<\/p><\/li><li data-start=\"3091\" data-end=\"3256\"><p data-start=\"3093\" data-end=\"3256\">Trabajamos la <strong data-start=\"3107\" data-end=\"3124\">propiocepci\u00f3n<\/strong>: el sentido del equilibrio y la posici\u00f3n articular, con actividades como el apoyo en un solo pie, primero con apoyo y luego sin \u00e9l.<\/p><\/li><li data-start=\"3257\" data-end=\"3381\"><p data-start=\"3259\" data-end=\"3381\">Incorporamos <strong data-start=\"3272\" data-end=\"3298\">plataformas inestables<\/strong> y <strong data-start=\"3301\" data-end=\"3316\">colchonetas<\/strong> para que aprendiera a adaptarse a distintos tipos de superficie.<\/p><\/li><\/ul><p data-start=\"3383\" data-end=\"3480\">Beatriz fue ganando seguridad paso a paso. Me dec\u00eda: \u201cSiento que el tobillo me obedece de nuevo\u201d.<\/p><hr data-start=\"3482\" data-end=\"3485\" \/><h2 data-start=\"3487\" data-end=\"3534\">Cuarta etapa: volver a caminar con confianza<\/h2><p data-start=\"3536\" data-end=\"3611\">La \u00faltima fase apunt\u00f3 a recuperar la funcionalidad en su entorno cotidiano.<\/p><ul data-start=\"3613\" data-end=\"3944\"><li data-start=\"3613\" data-end=\"3707\"><p data-start=\"3615\" data-end=\"3707\">Hicimos entrenamientos espec\u00edficos de <strong data-start=\"3653\" data-end=\"3672\">marcha adaptada<\/strong>, con cambios de ritmo y direcci\u00f3n.<\/p><\/li><li data-start=\"3708\" data-end=\"3803\"><p data-start=\"3710\" data-end=\"3803\">Practicamos situaciones reales: caminar en veredas, esquivar objetos, subir y bajar cordones.<\/p><\/li><li data-start=\"3804\" data-end=\"3944\"><p data-start=\"3806\" data-end=\"3944\">Reforzamos <strong data-start=\"3817\" data-end=\"3856\">estrategias de prevenci\u00f3n de ca\u00eddas<\/strong>, como el uso adecuado del calzado, la iluminaci\u00f3n en el hogar y la conciencia corporal.<\/p><\/li><\/ul><p data-start=\"3946\" data-end=\"4088\">La evoluci\u00f3n fue muy buena: en cuatro semanas el dolor hab\u00eda disminuido, y a las diez semanas ya caminaba con normalidad, sin bast\u00f3n ni temor.<\/p><hr data-start=\"4090\" data-end=\"4093\" \/><h2 data-start=\"4095\" data-end=\"4127\">Lo que aprendimos con Beatriz<\/h2><p data-start=\"4129\" data-end=\"4357\">Este proceso me record\u00f3 que <strong data-start=\"4157\" data-end=\"4249\">la rehabilitaci\u00f3n en adultos mayores no es solo f\u00edsica, tambi\u00e9n es emocional y funcional<\/strong>. Un esguince puede parecer simple, pero si afecta la confianza en el cuerpo, el impacto puede ser profundo.<\/p><p data-start=\"4359\" data-end=\"4409\">Algunos aprendizajes clave que quiero compartirte:<\/p><ul data-start=\"4411\" data-end=\"4729\"><li data-start=\"4411\" data-end=\"4469\"><p data-start=\"4413\" data-end=\"4469\"><strong data-start=\"4413\" data-end=\"4469\">No subestimes una lesi\u00f3n \u201cleve\u201d en personas mayores.<\/strong><\/p><\/li><li data-start=\"4470\" data-end=\"4563\"><p data-start=\"4472\" data-end=\"4563\"><strong data-start=\"4472\" data-end=\"4563\">El movimiento precoz, guiado y progresivo es la mejor forma de prevenir complicaciones.<\/strong><\/p><\/li><li data-start=\"4564\" data-end=\"4644\"><p data-start=\"4566\" data-end=\"4644\"><strong data-start=\"4566\" data-end=\"4644\">La prevenci\u00f3n de ca\u00eddas debe formar parte del tratamiento desde el inicio.<\/strong><\/p><\/li><li data-start=\"4645\" data-end=\"4729\"><p data-start=\"4647\" data-end=\"4729\"><strong data-start=\"4647\" data-end=\"4729\">Escuchar los miedos del paciente es tan importante como trabajar sus m\u00fasculos.<\/strong><\/p><\/li><\/ul><hr data-start=\"4731\" data-end=\"4734\" \/><h2 data-start=\"4736\" data-end=\"4785\">\u00bfQu\u00e9 hacer si sufriste un esguince de tobillo?<\/h2><p data-start=\"4787\" data-end=\"4876\">Si sos adulto mayor o cuid\u00e1s de alguien que lo sea, estos son algunos consejos pr\u00e1cticos:<\/p><ul data-start=\"4878\" data-end=\"5241\"><li data-start=\"4878\" data-end=\"4978\"><p data-start=\"4880\" data-end=\"4978\"><strong data-start=\"4880\" data-end=\"4919\">Consult\u00e1 siempre tras una torcedura<\/strong>, aunque parezca menor.
