Ten\u00eda 65 a\u00f1os, y aunque su actitud era positiva, me confes\u00f3 que hac\u00eda meses no pod\u00eda levantar el brazo derecho sin sentir un dolor agudo. Las tareas diarias \u2014como vestirse, cocinar o simplemente peinarse\u2014 se hab\u00edan vuelto frustrantes y, en sus palabras, la estaban \u201cdesgastando emocionalmente\u201d.<\/p><p data-start=\"779\" data-end=\"1024\">Hoy te cuento su historia, no solo porque logr\u00f3 una buena recuperaci\u00f3n, sino porque muchos pacientes con <strong data-start=\"884\" data-end=\"917\">desgarro del manguito rotador<\/strong> enfrentan lo mismo: dolor, miedo al movimiento y dudas sobre si podr\u00e1n volver a usar su hombro como antes.<\/p><hr data-start=\"1026\" data-end=\"1029\" \/><h2 data-start=\"1031\" data-end=\"1083\">\u00bfQu\u00e9 es el manguito rotador y por qu\u00e9 se lesiona?<\/h2><p data-start=\"1085\" data-end=\"1357\">El manguito rotador es un conjunto de cuatro m\u00fasculos que estabilizan y movilizan el hombro. En personas mayores, especialmente si son sedentarias o tienen enfermedades como <strong data-start=\"1259\" data-end=\"1291\">diabetes tipo 2 u osteopenia<\/strong>, estos tejidos tienden a debilitarse y lesionarse m\u00e1s f\u00e1cilmente.<\/p><p data-start=\"1359\" data-end=\"1634\">En el caso de Marta, el diagn\u00f3stico fue claro: <strong data-start=\"1406\" data-end=\"1453\">desgarro total del tend\u00f3n del supraespinoso<\/strong>, confirmado por resonancia magn\u00e9tica. Esto explicaba su dolor cr\u00f3nico, la <strong data-start=\"1528\" data-end=\"1564\">incapacidad para elevar el brazo<\/strong> y la <strong data-start=\"1570\" data-end=\"1589\">atrofia visible<\/strong> de la musculatura deltoidea y supraespinosa.<\/p><p data-start=\"1636\" data-end=\"1689\">Adem\u00e1s del dolor, presentaba signos cl\u00ednicos t\u00edpicos:<\/p><ul data-start=\"1691\" data-end=\"1877\"><li data-start=\"1691\" data-end=\"1738\"><p data-start=\"1693\" data-end=\"1738\">Disminuci\u00f3n importante de la movilidad activa<\/p><\/li><li data-start=\"1739\" data-end=\"1813\"><p data-start=\"1741\" data-end=\"1813\">Test de Jobe positivo (dolor e imposibilidad para resistir la abducci\u00f3n)<\/p><\/li><li data-start=\"1814\" data-end=\"1877\"><p data-start=\"1816\" data-end=\"1877\">Compensaciones posturales por el uso exclusivo del otro brazo<\/p><\/li><\/ul><hr data-start=\"1879\" data-end=\"1882\" \/><h2 data-start=\"1884\" data-end=\"1935\">Etapa inicial: calmar el dolor y ganar confianza<\/h2><p data-start=\"1937\" data-end=\"2102\">El primer objetivo fue <strong data-start=\"1960\" data-end=\"2005\">aliviar el dolor y reducir la inflamaci\u00f3n<\/strong>, lo cual es esencial para que el paciente se sienta seguro y dispuesto a moverse. Implementamos:<\/p><ul data-start=\"2104\" data-end=\"2309\"><li data-start=\"2104\" data-end=\"2173\"><p data-start=\"2106\" data-end=\"2173\"><strong data-start=\"2106\" data-end=\"2130\">Terapia manual suave<\/strong>, para relajar la musculatura periarticular<\/p><\/li><li data-start=\"2174\" data-end=\"2255\"><p data-start=\"2176\" data-end=\"2255\"><strong data-start=\"2176\" data-end=\"2203\">Electroterapia con TENS<\/strong>, \u00fatil para modular el dolor sin efectos secundarios<\/p><\/li><li data-start=\"2256\" data-end=\"2309\"><p data-start=\"2258\" data-end=\"2309\"><strong data-start=\"2258\" data-end=\"2284\">Crioterapia localizada<\/strong> al finalizar cada sesi\u00f3n<\/p><\/li><\/ul><p data-start=\"2311\" data-end=\"2594\">En paralelo, comenzamos a trabajar con <strong data-start=\"2350\" data-end=\"2372\">educaci\u00f3n postural<\/strong>: Marta deb\u00eda evitar movimientos repetitivos o levantar peso con el brazo lesionado. Este paso fue clave para que dejara de forzar la articulaci\u00f3n y entendiera que la <strong data-start=\"2539\" data-end=\"2593\">rehabilitaci\u00f3n tambi\u00e9n implica aprender a cuidarse<\/strong>.<\/p><hr data-start=\"2596\" data-end=\"2599\" \/><h2 data-start=\"2601\" data-end=\"2655\">Movilizaci\u00f3n temprana: comenzar a moverse con ayuda<\/h2><p data-start=\"2657\" data-end=\"2866\">Una vez controlado el dolor, pasamos a una fase m\u00e1s activa. A pesar del desgarro total, el cuerpo es sabio: otros m\u00fasculos pueden adaptarse para <strong data-start=\"2802\" data-end=\"2836\">compensar la p\u00e9rdida funcional<\/strong> si se entrenan correctamente.<\/p><p data-start=\"2868\" data-end=\"2882\">Empezamos con:<\/p><ul data-start=\"2884\" data-end=\"3157\"><li data-start=\"2884\" data-end=\"2935\"><p data-start=\"2886\" data-end=\"2935\"><strong data-start=\"2886\" data-end=\"2919\">Movilizaciones pasivas suaves<\/strong>, guiadas por m\u00ed<\/p><\/li><li data-start=\"2936\" data-end=\"3032\"><p data-start=\"2938\" data-end=\"3032\"><strong data-start=\"2938\" data-end=\"2984\">Ejercicios asistidos con poleas y bastones<\/strong>, para estimular el rango de movilidad sin dolor<\/p><\/li><li data-start=\"3033\" data-end=\"3157\"><p data-start=\"3035\" data-end=\"3157\"><strong data-start=\"3035\" data-end=\"3091\">T\u00e9cnicas de facilitaci\u00f3n neuromuscular propioceptiva<\/strong>, como el m\u00e9todo Kabat, para reclutar fibras musculares remanentes<\/p><\/li><\/ul><p data-start=\"3159\" data-end=\"3282\">Algunas semanas despu\u00e9s, Marta ya pod\u00eda <strong data-start=\"3199\" data-end=\"3251\">realizar actividades b\u00e1sicas sin dolor constante<\/strong>, lo que mejor\u00f3 mucho su \u00e1nimo.<\/p><hr data-start=\"3284\" data-end=\"3287\" \/><h2 data-start=\"3289\" data-end=\"3337\">Fortalecer sin da\u00f1ar: progresar con seguridad<\/h2><p data-start=\"3339\" data-end=\"3544\">Entrar en la etapa de fortalecimiento no significa cargar peso de inmediato. En un caso como este, donde no se repara el tend\u00f3n quir\u00fargicamente, se trabaja sobre lo que <em data-start=\"3508\" data-end=\"3512\">s\u00ed<\/em> est\u00e1 disponible funcionalmente.<\/p><p data-start=\"3546\" data-end=\"3559\">Incorporamos:<\/p><ul data-start=\"3561\" data-end=\"3866\"><li data-start=\"3561\" data-end=\"3627\"><p data-start=\"3563\" data-end=\"3627\"><strong data-start=\"3563\" data-end=\"3603\">Bandas el\u00e1sticas de baja resistencia<\/strong>, con progresi\u00f3n semanal<\/p><\/li><li data-start=\"3628\" data-end=\"3723\"><p data-start=\"3630\" data-end=\"3723\"><strong data-start=\"3630\" data-end=\"3656\">Ejercicios de esc\u00e1pula<\/strong> (como retracci\u00f3n y descenso), para mejorar la estabilidad proximal<\/p><\/li><li data-start=\"3724\" data-end=\"3809\"><p data-start=\"3726\" data-end=\"3809\"><strong data-start=\"3726\" data-end=\"3764\">Trabajo de core y cadena posterior<\/strong>, ya que una buena postura descarga el hombro<\/p><\/li><li data-start=\"3810\" data-end=\"3866\"><p data-start=\"3812\" data-end=\"3866\"><strong data-start=\"3812\" data-end=\"3866\">Ejercicios de coordinaci\u00f3n y ritmo escapulohumeral<\/strong><\/p><\/li><\/ul><p data-start=\"3868\" data-end=\"3982\">Esta fase requiri\u00f3 paciencia y escucha. No siempre el avance es lineal, pero <strong data-start=\"3945\" data-end=\"3981\">la constancia hace la diferencia<\/strong>.<\/p><hr data-start=\"3984\" data-end=\"3987\" \/><h2 data-start=\"3989\" data-end=\"4033\">Reintegrar funciones cotidianas sin dolor<\/h2><p data-start=\"4035\" data-end=\"4190\">Uno de los momentos m\u00e1s gratificantes fue cuando Marta volvi\u00f3 a atarse el corpi\u00f1o sin ayuda. Puede sonar banal, pero para ella era un s\u00edmbolo de autonom\u00eda.<\/p><p data-start=\"4192\" data-end=\"4212\">En esta etapa final:<\/p><ul data-start=\"4214\" data-end=\"4576\"><li data-start=\"4214\" data-end=\"4354\"><p data-start=\"4216\" data-end=\"4354\">Dise\u00f1amos <strong data-start=\"4226\" data-end=\"4264\">estrategias adaptadas para las AVD<\/strong> (actividades de la vida diaria), como t\u00e9cnicas para alcanzar estantes sin alzar el hombro<\/p><\/li><li data-start=\"4355\" data-end=\"4450\"><p data-start=\"4357\" data-end=\"4450\">Realizamos correcciones posturales frente al espejo para <strong data-start=\"4414\" data-end=\"4450\">evitar compensaciones cervicales<\/strong><\/p><\/li><li data-start=\"4451\" data-end=\"4576\"><p data-start=\"4453\" data-end=\"4576\">Incorporamos herramientas simples como esferas blandas, tablas de propiocepci\u00f3n y una rutina domiciliaria con recordatorios<\/p><\/li><\/ul><p data-start=\"4578\" data-end=\"4692\">Gracias a estos cambios, <strong data-start=\"4603\" data-end=\"4636\">logr\u00f3 independencia funcional<\/strong>, con <strong data-start=\"4642\" data-end=\"4691\">dolor bajo control y sin necesidad de cirug\u00eda<\/strong>.<\/p><hr data-start=\"4694\" data-end=\"4697\" \/><h2 data-start=\"4699\" data-end=\"4736\">Aprendizajes que me dej\u00f3 este caso<\/h2><p data-start=\"4738\" data-end=\"4871\">Cada paciente con desgarro total del manguito rotador plantea un desaf\u00edo distinto. Pero hay tres cosas que siempre me gusta remarcar:<\/p><ol data-start=\"4873\" data-end=\"5258\"><li data-start=\"4873\" data-end=\"5000\"><p data-start=\"4876\" data-end=\"5000\"><strong data-start=\"4876\" data-end=\"4914\">No todo desgarro necesita cirug\u00eda.<\/strong> Con buen acompa\u00f1amiento, muchos pacientes logran una recuperaci\u00f3n funcional adecuada.<\/p><\/li><li data-start=\"5001\" data-end=\"5127\"><p data-start=\"5004\" data-end=\"5127\"><strong data-start=\"5004\" data-end=\"5034\">La adaptaci\u00f3n es la clave.<\/strong> No se trata de \u201cvolver a lo de antes\u201d, sino de construir nuevas formas de moverse sin dolor.<\/p><\/li><li data-start=\"5128\" data-end=\"5258\"><p data-start=\"5131\" data-end=\"5258\"><strong data-start=\"5131\" data-end=\"5189\">La educaci\u00f3n y la motivaci\u00f3n sostienen el tratamiento.<\/strong> Sin confianza en el proceso, no hay plan de ejercicios que funcione.<\/p><\/li><\/ol><hr data-start=\"5260\" data-end=\"5263\" \/><h2 data-start=\"5265\" data-end=\"5318\">\u00bfEst\u00e1s enfrentando un dolor de hombro persistente?<\/h2><p data-start=\"5320\" data-end=\"5567\">Si te duele el hombro hace tiempo, evit\u00e1s moverlo o te cuesta hacer tareas cotidianas, <strong data-start=\"5407\" data-end=\"5428\">no lo dejes pasar<\/strong>.
